These tools and resources can help you support caregivers and their patients:

  • Advance Care Planning resources – tools to help patients and caregivers have discussions about end of life care
  • CANHELP Tool
    The CANHELP (Canadian Health Care Evaluation Project) questionnaire was designed to help researchers, health care providers, patients and their families better understand satisfaction with care provided to seriously ill patients with advanced disease and to identify opportunities for improvement. Initially used by researchers to measure end of life care satisfaction, the questionnaire is now also available for health care providers, patients and families as a catalyst for discussions about treatment, to identify opportunities for improvement, and to measure quality of care delivered by a program or institution.
  • Canadian Virtual Hospice  – has forums and an Events Calendar of conferences and courses for health professionals on end of life care
  • Initiative for a Palliative Approach in Nursing: Evidence and Leadership (iPanel)  - research for nurses, by nurses.
  • Pallium Project  - Since 2000, the Canadian Pallium Project has promoted collaboration, coordination and communication for access, quality and new capacity in service of the seriously-ill and dying at the community-level throughout Canada. Resources include iTunes podcasts and videos as well as Youtube videos.
